HCC is looking for a Bachelor level Social Worker to join our Maternal Infant and Health Program (MIHP).
This person will function as a multi-disciplinary team member performing assessments, formulating care plans, providing education and teachings, and managing referrals.
The MIHP Social Worker will conduct home and office visits.

Manage internal and external referrals:
describe program and schedule assessments.
Perform maternal and infant assessments.
Make any emergency referrals as needed based on initial assessment.
Responsible for maintenance and care for a patient panel of approximately 50 patients (at any given time).
Mental health focus (i.

, advocate, assist, refer and follow up on referrals with the patient).
Counseling referral source for nurse on any mental health needs.
Attempt to ensure maximum reimbursable visits per year.
Set productivity goals.
Attend and participate with team meetings.
Schedule patient appointments (home, virtual, office visits) Documentation requirements within MIHP Electronic Medical Record (Ross).
Communication with HCC staff regarding patients within EMR.
Attend mandatory State meetings.
Job
Qualifications:
Possess a Bachelor level Social Worker degree from an accredited school.
Maintain current, in good standing, licensure from the State of Michigan.
Maintains BLS certification.
Two years of experience providing comprehensive services to families.
Ability to evaluate and judge situations continually.
Ability to problem-solve.
Ability to work independently and within a team.
Possess excellent organizational and interpersonal communication skills.
