RESPONSIBILITES:
Develop, plan, implement and evaluate service program and service in accordance with state and federal guidelines.
Identify and provide for resident's social, emotional and psychological needs Provide service coordination
Qualifications:
Licensed Social Worker, preferred One year of supervised social work experience in a healthcare setting, preferably with the psychosocial of the elderly Working knowledge of nursing homes regulations, including MDS procedures, and reimbursement and eligibility guidelines under Medicare and Medicaid Excellent communication skills; written and verbal Positive Can-Do Attitude Excellent Attendance
